form field values passed from email

I will be sending emails to existing leads for a survey solicitation. The email will include a custom field value that is the unique identifier for the lead. 

If a lead clicks through the email to fill out the Marketo form (survey), is it possible for the form to capture the custom field value that was included in the outbound email?
