Competitors will sometimes download product trials for research purposes, so we'd like to block certain domains from ever receiving emails, including operational emails. Is this what sets blacklisted apart from the unsubscribe or marketing suspended fields?

Black listed leads do not receive operational emails. Marketing suspended leads do.

Are they excluded from triggering Alerts as well?

Blacklisted means that THEY are blocked from receiving communications. So if, for some reason, you have an alert that would go to a record that has been blacklisted, then they would not receive that alert.
But I think you're asking if a blacklisted lead can trigger an alert to be sent to someone else, like an internal record (ex: sales rep). In this case, I would say yes, A blacklisted record can still pass through all the flow steps. They just can't be the ones to receive the communication.
