Adding Lead Creation Date to Lead Report

I've created a lead report with a Smart List that filters:

• Filled Out Form
  • Form Name: Contains: Google
  • Date of Activity: This Year
• Sales Status
  • Sales Status: Is: Qualified

The Setup is:

Lead Created at: This year

Group Leads by: Google Campaign ID

However, when I generate the report, I would like to see a column of when each lead was created. Can someone help me to accomplish this?

group by month and then Created At is this year.

Thanks for the reply, but I would still like to see the Google Campaign ID as well. Thoughts?
