"Clear Form" as link or button option in FORM Builder 2.0

Sometimes it happens that forms are pre-poulated with the details of someone else or that you wish to subscribe a second person to f.e a workshop. 

To eliminate the chance to overwrite the data this "clear form" option would be handy.

I have now implemented this "clear form" action thanks to the script I found in this thread https://community.marketo.com/MarketoDiscussionDetail?id=90650000000Pil2AAC

But it took me a while to get it right and I guess not everyone knows how to tweek java and JQuery scripts 😉
