This is more Discussion than Idea, but in general it is pretty accurate. It get's tricky for companies that are also ISPs and also outside of the US, but we by a database that looks up who owns the IPs.
The field Infered is for leads you know and for anonymous.
The accurate for the lead depens on the company setup of ISP. In denmark this field will give you correct data for maybe only 50% of the data coming in.

Ask you self this. does your Company have IT department that setup the ISP when your surfing.
I agree with Jan. We see about as many ISPs as we see actual company names. Add to that the fact that many people are using mobile devices for their surfing and you'll get a lot of AT&T something or others as well.
This has made me discover just how challenging reverse IP lookups are!
