I frequently upload lists that have multiple columns of specific answers to specific questions. It's not worth creating fields for each question, instead is better to append them all to a Person Notes field that goes to SFDC for Sales to see. Unfortunately List Import doesn't allow the header to be captured on any field. But as long as I have a String or Text Area field, a checkbox to "Include Column Header" would save me editing multiple fields every time.
