Could be a browser or internet session issue. Marketo is trying to save everytime you make a change. But after a while if you have been working in one tab of your browser all day, may need to refresh your browser from time to time.Raise a ticket with support with screenshots.

From within the program, were they give a progression status?Also, Is the errors on the leads activity history from SFDC?If there is a mis-match of progressions/Status between Marketo Program and SFDC Campaign.

You can create a new lead for contacts by assigning to a SFDC queue. Contacts can't be owned by queues so Marketo would create a duplicate record on the lead object.
