I take it that based on the package/service paid with BrightTalk, customizing their form will cost extra? I'm not sure of any other way other than URL prams.Interesting to see what else can be used as a work around.
The function is there.Make sure you have all the fields on your form. (Your Opt in check box and Country Picklist Field)Select Opt-in field and click 'edit Visibility rules' on the right side. Now create your logic - show Opt-in when Country field with value equals Canada to Show.
If the field is being used currently on a form, the Marketo admin team will get an email about a change made in SFDC of a field that Marketo uses.

the frozen part to your issue, could be that you have the text layer (above the form) hanging down over the first few fields.For the stretching part our your issue, could be related to your CSS on your template.

Sounds like you have a delay starting to happen. Marketo could be struggling to keep up with changes in your SFDC. I've also ran into this situation in the past and it was due to an API delay with major changes in our SFDC.

Its how you have set up that field on the form. Is it one value 'yes', as the slide deck?Once you have set it with one value, it will populate with your yes value to select.
