I spoke tooooooooo soon. I tried to resubmit the form and it's not submitting.
It does exist. See the screenshot. I changed the code back to "marketo-xdframe" and then cleared my cache and it's working now. Thanks for the write up. If I had cleared my cache I would have resolved this yesterday.

Thanks for the reply, Sanford. Is there anythat can be done so that campaigns are not blocked? With forms, we've changed the domain in the javascript and they now show up when ad blockers are turned on.

It's not preferable, but I was able to hide the tag using a comment tag. I don't know the implication, if any, of doing that, but it worked.
