Smart campaign trigger not fire "fill out form"

Hi, I have a program with a smart campaign which trigger once a lead fill out a form - it's not working. The form is Marketo's placed on external landing page. When I do the test, I see the "fill out form" in the log, but no smart campaign is triggered.  I used the "fill out form" trigger and nothing more.
