White Paper Download and Sample Request Form

Hello,
We would like to create a form for a recipient to request a white paper download and a sample if desired.  My dilemma is that I do not want to ask for someone's address if they are just requesting the white paper.  Is there a way to create a form where if they check the box to receive a sample in the mail that the address field would appear as well as the address field not appearing if they do not check the sample in the mail box?

Hi Nicole, you can make a single form and use the address fields conditionally. That is to say, that you could have them show only if "sample" is checked. This guide should point you in the right direction as to how you can make fields appear/disappear based on other selections.
