Survey in Marketo

I want to create a simle survey in Marketo to track the experience of custoemr who have ordered and those who have got a quote but not ordered.

How do I create these survey on Marketo?

I want to send gift cards to users once they complete thsese surveys.

We use SurveyMonkey for our surveys, and use a Marketo landing page with custom form on the survey thank you page. We provide a giveaway, encouraging them to complete the quick form after the survey.

In the absence of a true survey tool, I usually create custom fields in Admin > Field Management.  

I add these fields into a custom form, and plop that on my survey landing page.
