"Don't let them unsubscribe" code not working

I tried using the codes provided on this page for our subscribtion page & it is not working.

I am trying to seperate the "unsubscribe all" from the other listed items with a line as well as use the function that when they select unsubscribe all they all the boxes are checked.

Hi,

We just went through this same issue at our company. 
Looks like the script doesn't work with Forms 2.0  but did work for us with the older forms. 
Try that if you haven't already.
