Multi Select Check Boxes

I've followed the instructions for this community discussion (https://community.marketo.com/MarketoIdeaDetail?id=08750000000HNQVAA4) which pointed me to this article on how to create multi select check box: http://test.hollebone.ca/marketo-form-convert-multi-picklist-to-checkboxes-with-jquery.

I've followed the instructions, but I'm not able to submit the form. Does anyone have any ideas how to fix this? Link to test page: http://go.appsense.com/MarciaTest_webinarpage1.html.

I would like to access the article on http://test.hollebone.ca/marketo-form-convert-multi-picklist-to-checkboxes-with-jquery. as I'm trying to create checkboxes for a multiselect list. The page will not load for me...can anyone provide those instructions? Thank you.

You seem to have some Javascript errors from other things on the page. Might be worth looking into that as a starting point.
