Convert a multi-select box to checkboxes

Does anyone know of a solution to convert a multi-select box to checkboxes?

Here's a cached version of Eric Hollebone's code that supposedly did this, but I've never been able to get it to work. Good luck.

Thank you, that's close but is there any way of having the checkboxes not inside a pull down list?
