Thinking about storing cookie consent in DB some more, it would seem you need to post this to Marketo as soon as it's set via banner/pref centre or upon form submit for an unassociated lead once they become known. Simple enough if using a custom banner and I assume achievable if using OneTrust (which appears to be the most prominent third-party vendor in the space) by hooking onto their cookie.
One risk I see is if the person comes back and toggles their preference in an unassociated session then their cookie consent preference (at least in that browser) would be out of sync with the preference recorded in Marketo unless they submitted another form.
