Yes, it's called "StoreForest-red-LightFG-LightBG-layout2"

Yes, it's part of our instance.  Not sure why Marketo can't send you the file - all they would need to do is copy the HTML and place it in a text file.  I can do that for you.  Just let me know how you want me to send this to you.  Too bad there's no way to PM members on this community (or attach files to message threads).
