It has appeared again now for me as well. We've just been having this issue for months where forms will not show up on pages for random people both internal employees and external customers. I just cannot seem to find a pattern or reasons for random webpages to not be displaying forms. I'll just keep troubleshooting with my web team for now but thanks for your help!
