Marketo form incomplete labels

I'm creating some marketo form surveys and am running into an issue with the question labels. My labels display all the text on the editing page, but when I preview my survey, only the first sentence of the labels shows up. Any ideas on how to fix this? I tried making the question required vs not-required and that didn't help. 

For example, on the given link, the question, "When do you plan on moving" is supposed to display, "When do you plan on moving? Fill out to the best of your ability or leave blank if unsure."

Certainly not, that's a secure link. You have to publish an LP.

It seems I may have misunderstood the capabilities of marketo forms. Will I be able to integrate my marketo forms into emails sent through marketo or can these forms only be placed on websites?

Marketo forms aren't to be used in emails. But no HTML forms work across email clients, this is not a Marketo-only restriction. There is a smattering of support for form inputs in HTML email, nothing you could ever rely on. You should be sending people to an LP.
