Hi, I've got some icons in an email that look fine to me. But my client claims they are distorted vertically, We've tried using their screen resolution to replicate it but can't seem to find an issue.
Any ideas on how to ensure everyone see's the images in the same 'ratio'?
Have you tried a tool like EmailonAcid? Depending on how you build them out, different email clients and a variety of other factors will affect how an email is presented.
I haven't, just checked it out and it seems quite expensive to 'just check
my icons' There's no way for me to find out how my email is going to look
in various clients in Marketo?

I'm with Chris on this one. A tool like Email on Acid or Litmus is essential to make sure your emails are going to render the way you want them to on all clients and devices.
This can happen, especially when emails are forwarded.
Make sure your icon images are the exact height and width you want, then specify the height and the image in your css.
Do Litmus test where you can get the idea of how the email is rendering on different devices.
