I believe there is problem happening now on the Smart List related to the form. The list of contacts whom had previously submitted the form is not showing on my smartlist.
I am unsure if the rest of the users are having the same issue.
Please check.

Let's get more (a lot more) info. Not much here at all!
Show the Smart List filters, and show the Activity Log entry for a lead you believe should qualify for the SL.
