Getting field values to show up in alert emails

I created an alert email so a few members of our staff are notified when a user completes our Pricing form at casthighlight.com/pricing

Right now the alert looks like this:

This user just submitted a purchase request for CAST HIGHLIGHT.

How do I adjust this so that the alert populates the user's actual selection for subscription plan and length and shows that they checked the service agreement? So instead of 1s we might see Department, 30 day, "Yes, read and agreed to CAST MSA"

Do I need to create tokens?

Hi David,

Could you clarify what the lead's field values are, and what the field types are? That will help clarify your best course of action.

The first 2 are radio buttons. Field values (stored and display) are Department, Mid-size, Corporate; AND 30 day, 12 month.

The third one is a checkbox with a stored field value of yes. Currently the display value is blank.

Did you ever get a response for this?  I am wanting the same thing for registration which are checkboxes. 

My thoughts:  When a user registers for multiple newsletters, I want to send a confirmation email to that person confirming what Newsletters they signed up for, but all I can get to populate is :
