Embed lightbox form on non-marketo page

Is there a way to get a header for a form to show up in the lightbox?  

I would like to embed one of our forms on a Wordpress page and use a lightbox. The test page I made works well, except that the only part that shows up in the lightbox are the fields and button.  I can make a header on the Wordpress page, but it looks funny without it showing up in the lightbox.

Anyone know a way to do this?

If you add a rich text box to the top of the form, that's your header and it works fine in the lightbox: http://jsfiddle.net/sanford/h6yxs7e2/
