Hi everyone,
I am creating a form with custom fields.  Which field type should I use to create and report on a field with multiple selects?

Do you mean which field type you need to assign in the Form?

You can set a Select or Multi Select.

The thing is that Marketo separates these values with a semi colon in the actual field, so you would need to use a filter in the smart list that uses CONTAINS or has the complete line of possible combinations. If you go in there, you'll see what I mean.
