Checkbox not working on live URL - HELP

Hey all:

I created a new form and placed it on a landing page. In the form preview the check boxes work, however when live they do not (here is the link for your reference). Any idea what might be going on?? Your help is greatly appreciated. Thank you!

Your problem stems from having 2 Marketo forms (your form ids 1341 and 1342) both in the page. This wreaks havoc when you use a CSS theme that replaces the checkboxes with "fancy" checkboxes (I happen to think they're not as good-looking as the native boxes!).

Since only #1341 is actually visible, I recommend you conditionally include only one on the page. That will solve the prob.

Hi Sanford- I tried removing the second form and just having the desktop version, however that did not help. They still don't click? Thoughts?? What am I missing? Thank you

Both forms are still there. Maybe you didn't reapprove.
