Value List Issue - One value showing up multiple times

We've got a form that has a drop down that includes four options:

Option A
Option B
Option C
Option D

While Option D is one of the options in this field in Marketo/SF, we do not want it to appear as an option for this particular form. So I removed Option D from the value list in the form properties. 

However, we are now getting this list appearing in the drop down on the live version of the form:
Option A
Option B
Option C
Option A

Any ideas on why this is happening or how to fix it?

Did you approve the landing page after you changed the form?  The form change will not take effect until you approve the new landing page draft that was automatically created when you modified the form.
