Issue with visibility rule

Hello,

I have created a new form that is using exact same settings as an existing form.
In this new form, I have 2 visibility rules:

- If country = US or Canada, display State or Province.
- If country is not US or Canada, display Industry

The rules are working perfectly in the original form. The settings look to be the same. Preview is perfect.

However I have tried in 3 different browsers, form is not working as it should and I don't understand why!

Hello,

I was told that issue comes from the new template created by our designer team. Apparently there is a custom code that prevents the visibility rules to work properly. We need to look into issue.
