how to create a new campaign and a new form in it and a flow to set form action?

The next steps are:
- embedding the form in the website
Select the form, click Form Actions and then Embed Code.

- set up a triggered campaign to erspond to form fill outs

I have created a test program , test campaign and a test form (local asset).

Now when i click on test program i can see testform below unused Local Assets, Now here i want to merge a test form with test campaign and then In campaign, use flow to set form action and take embed code to www.abc.com.
