First scenario:

Second scenario:
1 banner image, 1 hyperlink-ed text, 1 button --> all going to different URLs.
How would Marketo track the Clicks to Opens ratio in these two cases?

Simply add a dummy URL parameter to your various CTA going to the same URL in case 1so that they can be differentiated. Otherwise, they will be combined in Marketo reporting.
For instance, https://my.marketodomain.com/mylandingpage.html?id=1 and https://my.marketodomain.com/mylandingpage.html?id=2
