How do I create a form with multiple choice questions and embed it in a page?

I would like to create a form which contains one multiple choice question. Does anyone know how I can do this? I would also like to embed this form in a page and don't know where to get the code from.

Hi Carolina,

If you have existing field you can use that for your question or else create a new one to keep track of your questions and answers. Make sure in your Marketo form you have selected the field type as checkbox, under Values click edit to add your filed values.
