Issues integrating Demandbase and Marketo?

Hi All,

I've tried integrating our Marketo forms with Demandbase by adding the Demandbase tag and hidden fields we'd like captured when a lead fills out a form. We're still having issues having information carry over from Demandbase to Marketo when the lead is created. Here is how I've created the hidden fields:

Does anyone have any experience using Demandbase with their forms and can provide any suggestions for why this issue might be happening?
