The champions group is not the place to post product questions. See Getting the most of the community when you have a Marketo question. In champions only very few people will see you post and you are less likely to get answer (Although you lucky as I was precisely online )
So you should move this one to the product section.
The way it is designed, the flow will be executed only if people are a member of the eComm test Engagement program. So the first thing I would suspect is that the lead who is filling out the form is not a member of this EP.
The second place to look at is the qualification rules. Are you sure they are not set to "run only once"?
Thanks for the reply. Moved the topic. I have tested about 10 times with every member of this EP test - they are certainly members. I also have trigger campaigns set up the same way that are successfully triggering for things like if opens email and member of engagement program, add to lead score. I go through the activity logs and all this smart campaigns are successfully triggering except this.
Qualification rules allow leads to flow through every time.

Are you seeing the fill out form activity in the activity log of the test lead? Are you sure the list of 8 forms in the trigger do not have a spelling or the form you are testing is in this list of 8?
Sorry for asking obvious things, but aside of these there is no reason why the trigger would not fire and you then should call in support. There not much more we can do from the community.
