I'd create that report in SFDC, because it's more like a sales management report.
Josh - Best would be to create a Smart List, since through Smart Lists you can see all the data you mentioned. No way to receive a Smart List through e-mail. If you do have the RCA platform, you could create the report you mentioned, and have it e-mailed to you regularly.
If you do not have the RCA and are looking to have a report e-mails to you, you could create a Lead Performance Report with the relevant criteria, but it's going to be in a summarized format (i.e. grouped by a certain attribute such as Lead Created Date, Country etc.) and you're not going to see all of the details you mentioned in a list format.
