Many customers have a couple customer scores (like Demographic and Behavioral) that they increment. They use "Lead Score" as a total and add to this whenever they add to the other lead scores.
So the default "Lead Score" is a sum of all the other custom scores. This way, you can have an idea of total overall engagement as well as have the granularity to see what they did to achieve that score.
We currently do not pull in the "attention score" as part of the sync from GTW. You could possibly use the API to populate that score in Marketo, or upload a list with that score. You would need to create a custom field in Marketo to store the value.
