From the standpoint of the USA CAN-SPAM law, your unsubscribes are unsubscribed from everything unless they fill out a form requesting you communicate with them again (e.g. download request on your web site).
I would try to provide the ability to unsubscribe from specific communications as well as everything.

Thanks for your reply Elliott. I did check out the webinar, which prompted me to update our unsubscribe page and process. I'm trying to figure out the best way to make the switch from the old format to the new one. I do agree that all previous leads that have unsubscribed should remain unsubscribed.

You need to keep the Unsubscribed field on there somewhere. I usually recommend keeping this as an Unsubscribe All option at the end of the form so that they can choose other communications or channels first. There are some examples on the second link I provided.
