I've had that issue before - the user is still sitting somewhere in the back end. You'll need to go to Marketo support and ask them to clear it so you can reinstate the login. It's a nightmare!

That depends on whether you want to (a) delete them from your Marketo instance, or (b) remove them from your list, but leave them in Marketo.
