Unsubscribe Capturing

I have a  issue with Unsubscibe here is the case:

There are 43 leads who click on Unsubscribe form and submitted but I see the status "Unsubscribe = True" for only 4 leads, also the Unsub report shows only 1 lead.

Any idea or suggestion here?

Can you tell us how you are pulling the data? Smart list? Report? Page Report? Results of campaign?
