One of the most frequent reason why the campaign member insertion fails is because Marketo cannot create the lead in SFDC in the first place, because some lead data is missing or incorrect.

Therefore, it is quite recommended that change program status flow steps be always preceded by a wait step so that other (i.e. data management) campaign have enough time to run and fix the data prior to insertion into the program.
