Is there a way to have 2 page form?

Is there a way to set this up in Marketo without having to create two forms? 

So Ideally Page 1 
 

Name 
Email
City
How Did You Hear About Us? 

And then a button that they hit that says "Next"

And then on the Second Page 

Phone Number 
DOB
etc...

And then on this page a CTA Submit button


Is this possible in Marketo?  the fields above are just example fields 

This is going on a Non-Marketo page.

You may be able to use a single progressive profile form. On the first page would be the first set of fields and a submit button labeled Next that would submit with what was captured. On the second page the progressive fields would be all the remaining fields.

My vision the way I described it was to use a single form asset twice, but you could go either way. The first pass would capture the first set of fields needed and link to a follow-up page that would capture the second set.

Jerry's thinking is right, just depends on how much information you already have about the prospect. Say I have already completed a form and you have my name, email, and city. If you use Progressive Profiling, then the 4 questions I would be prompted with would be how did you hear about us, phone number, DOB, and the next one in your sequence. Then, when I go to the next page, the next 4 fields in the sequence would show. 

If you are good with that, then you would use one form and have 3 landing pages. The first two would have the form on it. The third would be just your general thank you for participating. You can customize the landing pages however you would like.

If you are not, then you would create 2 forms with the specific information you want to gain from each one.

Is this progressive profiling designed for more than one visit though?  

I was hoping to have a nex button at the bottom of the form and then go to the second landing page with the second set of form fields (like how did you hear etc.)
