How do I create a form with multiple questions?

We want to send an opt-in survey to all of our leads with several survey questions such as interests and region. How do I add multiple questions to one form?

Do I need to create the fields in Salesforce include the selection options? For example. Field= region, Selection Options= CA, NM, NY, NJ

Do you want the data to be seen by your Sales team? If not you can just create custom fields within Marketo, and avoid having any unnecessary fields in Salesforce.

Are you planning on using this data to segment people or trigger lead nurture campaigns? Reason I ask is sometimes its easier to just use a survey tool like Survey Monkey to conduct surveys, unless you are using the data to enrich your database. Survey Monkey has lots of options to customize the survey and make it easy for people to respond.

If you are planning on using this data to help you segment your database, and the sales team would benefit from this data you can either create new fields in SFDC or Marketo.

We are planning on using the data to segment our database for email.  Where can I create new fields in Marketo? I didn't see that option when trying to create the form. If I create the fields in Marketo, how will it speak to Salesforce? For creating Smart Lists, would it make more sense to create the fields in Salesforce first?

In that case, you can create new fields. You just want to double check that those fields don't already exist somewhere in your database.

If you want to create them in Marketo, you want to go to admin, and then field management. From field management you select 'New Custom Field' and then select the field type.

You want to also make sure that you name it correctly the first time, as once you create a field it is in there for good (although you can 'hide' it).
