Person should qualify for smart campaign, but didn't?

I have a smart list that looks like this:

I had a Person fill out a form (should qualify based on top trigger):

Here is their relevant qualifying information:

But, they didn't end up in the campaign. All the information should have been filled/submitted at the moment of form submission, so it shouldn't be a timing thing. Other people have successfully run through this flow recently as well. Any ideas/thoughts?

Hi Andrew,

If the "person source", "converting source" and "converting medium" filters in the campaign are set through the form or worth, through another "fills out form" campaign, then they is a risk that the values do not qualify at the exact moment your "fills out form" trigger is fired. This is due to how Marketo processes data updates, posting them to the database with a slight delay before they can be reevaluated.

This is why Sanford is asking for the details activity log, to see the exact order of operations.
